Hempstead ISD to Host Childhood Immunization Clinic on Nov. 30

Hempstead ISD in partnership with the Texas Department of State Health Services will be providing free childhood immunizations for Hempstead ISD students. The event will take place from 10 a.m. to 3 p.m. on Wednesday, November 30, at the Hempstead High School Football Field House (located behind Bobcat Stadium). A parent/guardian must accompany their child and bring a copy of their shot

record.

Immunizations are the most effective way to reduce the incidence of serious vaccine-preventable diseases. A student must be fully immunized against certain diseases or must present a certificate or statement that, for medical reasons or reasons of conscience, including a religious belief, the student will not be immunized. For exemptions based on reasons of conscience, only official forms issued by the Texas Department of State Health Services (DSHS), Immunization Branch, can be honored by the district. This form may be obtained by writing the DSHS Immunization Branch (MC 1946), P.O. Box 149347, or by visiting corequest.dshs.texas.gov. If your child received any immunization, please bring the shot record to the campus school nurse so the student’s record can be updated.
